Avoiding having people stand behind you can't always be avoided but one can minimize it. One just has to go through the effort to avoid it. I do it often. So it can be done. Same with carrying a gun. Can't always be done but do it as often as you can. In self defense there is never a guarantee of survival. We just do what we can to increase the odds of survival.

If it was a major arterial bleed, most of us are not trained on using a finger to stick down into the artery. I’m a former EMT and have never been trained to do that, yet there are numerous real-life encounters of that technique saving a person. Something to think about.
