Well… they shouldn’t be. The vast majority of headbutt stories generally involve a person willingly remaining in a position to be headbutted despite indicators that they are in danger. Consensual Fights or Lethwei matches are very different scenarios.

I know this is gonna seem stupid, but you don't want to go too low with your head. Even tough frontal-parietal seam becomes thicker with age, a good smash, eg a headbutt, can cause a lot of damage. I'm a short guy in local terms [175cm, tanslates to 5'9'' I think… 185/6"1'' being average], and a few times I got a topside headbutted in training, hurts like a mofo. I even made a guy sit down on his but when I pulled it off, cause he tried to come in under my guard.
Headbutts can do a lot of damage, just youtube headbutts, there are plenty of people who have been KTFO by a headbutt. Some people have thick foreheads, and they use that to their advantage. It is an explosive attack that the victims rarely see coming.

Also head butt defense: grow a big beard to act as a cushion, then block everything with your chin
