I use to work at a detention center and our policy for pregnant ladies was to hand cuff through her front, because of liability purposes if she fells on her stomach.
Based on the last position the initial officer was in and based on the circumstances, I'd say a knee to the face would have been great at bringing her into compliance and would have been completely reasonable.

Of course you could just make it policy that an officer must be constantly supporting a pregnant woman when cuffed in the back, in case of a fall and reduces the danger of being cuffed in the front. The ultimate strategy around this, is dont break the law! But, thats really hard for some people to follow.
